At United Care of Massachusetts, we provide HIV/AIDS Care Management that centers your health, dignity, and quality of life. Whether you’re newly diagnosed or have been living with HIV for years, we’re here to support you in staying healthy, connected to care, and empowered in your journey.

At United Care of Massachusetts, our Transgender Care Management program offers compassionate, respectful, and individualized support for transgender, nonbinary, and gender-diverse individuals navigating the healthcare system.

The Central Mass Providers Network (CMPN) is a collaborative coalition of healthcare, behavioral health, and community-based organizations dedicated to improving the quality, coordination, and equity of care across Central Massachusetts. Through shared resources, integrated referral pathways, and strong cross-sector partnerships, the network supports providers in delivering seamless, whole-person care.
CMPN serves as a regional hub for communication, problem-solving, and innovation—helping agencies work together more effectively, reduce barriers for clients, and strengthen the overall system of care. By bringing providers to the same table, we work toward a healthier, more connected future for every resident of Central Massachusetts.

The Central Massachusetts Behavioral Health Network strengthens regional behavioral health systems by uniting mental health providers, improving care coordination, and expanding access to equitable, person-centered support. We work collaboratively to address gaps, share resources, advocate for community needs, and ensure that all individuals in Central Massachusetts can access the mental health care they need to thrive.

The Central Mass Community Support & Social Determinants Network (CM-CSSDN) is a regional collaborative of community-based organizations, housing programs, food access providers, employment services, care navigation teams, and health equity partners working together to strengthen the non-medical supports that shape everyday health. By improving coordination and communication across systems, the network helps individuals and families access essential resources more quickly and effectively.
